Chewy, I have not personally used the Hyles Interleaf you mention, but I have heard it is good. Cambridge used to do one but dropped it several years ago, sorry I cannot be of better help.
Beth. I have a Cambridge Cameo Wide Margin there are still a few 'knocking around' here in the UK. However you can get a Concord Wide Margin now, if you are looking for a Loose Leaf, I know Riverside used to publish one with a leather Cover with post type fixings. Hendrickson publish both a Wide Margin and a Loose Leaf Bible.
